To receive your CE Certificate or Certificate of Attendance, both the Evaluation Form and Contact Hour (CE) Summary Form MUST be completed

The Contact Hour (CE) Summary Form MUST be thoroughly completed and submitted by June 30, 2025 to avoid a late fee charge.
